“The ruling on aborting a 20-day pregnancy.”

Alhamdulillah, my first child was born by C-section in February 2026. After the delivery, the doctors strongly advised me not to become pregnant again for at least nine months in order to protect my health and to improve the chances of having normal deliveries in the future. They recommended that there should ideally be an interval of approximately 18 months between the births of the two children.

My husband and I wish that Allah Almighty bless us with several children. However, according to the doctors, repeated C-sections, particularly more than four operations, can pose serious risks to the mother.

Furthermore, Alhamdulillah, I am currently approximately 20 days pregnant, while only about four months have passed since my previous C-section. According to the doctors, because of the short interval between the two pregnancies, this pregnancy is considered high-risk. There is a risk of uterine rupture, which could potentially endanger my life and could also affect or permanently impair my ability to have children in the future.

In addition, I still experience pain around the site of my previous C-section incision. This makes me concerned that my body may not yet have fully recovered from the previous operation.

In this situation, considering that the pregnancy is still at a very early stage (approximately 20 days) and that the above-mentioned medical risks are present, is it Islamically permissible for me to terminate this pregnancy? For example, would it be permissible to take medication or tablets that would cause the pregnancy to miscarry?
I would be grateful for guidance on this matter in light of the Qur’an, Sunnah, and Islamic jurisprudence (Fiqh).

الجوابُ حامِدا ًو مُصلیِّا ً

In the situation described in the question, based on the reasons mentioned by the questioner, she may have the pregnancy terminated before completing four months. However, it is not permissible to terminate the pregnancy after four months have been completed, and this must therefore be avoided.

مأخَذُ الفَتوی

فی الدر المختار : «ويكره أن تسقى لإسقاط حملها … وجاز لعذر حيث لا يتصور۔۔۔۔وإن أسقطت ميتا ففي السقط غرة … لوالده من عاقل الأم تحضر» الخ
و فی الشامیۃ : تحت «(قوله ويكره إلخ) أي مطلقا قبل التصور وبعده على ما اختاره في الخانية كما قدمناه قبيل الاستبراء وقال إلا أنها لا تأثم إثم القتل (قوله وجاز لعذر) كالمرضعة إذا ظهر بها الحبل وانقطع لبنها وليس لأبي الصبي ما يستأجر به الظئر ويخاف هلاك الولد قالوا يباح لها أن تعالج في استنزال الدم ما دام الحمل مضغة أو علقة ولم يخلق له عضو وقدروا تلك المدة بمائة وعشرين يوما، وجاز لأنه ليس بآدمي وفيه صيانة الآدمي خانية (قوله حيث لا يتصور) قيد لقوله: وجاز لعذر والتصور كما في القنية أن يظهر له شعر أو أصبع أو رجل أو نحو ذلك الخ (کتاب الحظر و الاباحۃ فرع: یکرہ اعطاء سائل المسجد ج:6 ص:429 ط: سعید۔کراتشی)۔
